This monthly service is designed to offer extra support to patients living with a single, serious chronic neurological condition that requires consistent care.

What Is Principal Care Management?

PCM is a Medicare- and TennCare-approved program that provides ongoing, personalized care outside of regular office visits. It’s ideal for patients managing conditions such as:

  • Multiple Sclerosis (MS)
  • Parkinson’s Disease
  • Chronic Migraine
  • Epilepsy
  • Other serious neurological conditions

Through PCM, our dedicated care team helps you stay on top of your condition — proactively, not just reactively.

What You’ll Receive

By enrolling in the PCM program, you gain access to:

 A team member will reach out each month to monitor your symptoms, medications, and progress.

We help schedule appointments, manage referrals, and track labs so nothing gets missed.

Enrollment in PCM is completely voluntary. Once you opt in, we will document your care activity monthly in your medical record. You may opt out at any time.

Billing Details

PCM is covered by Medicare and TennCare, and may show as a small monthly charge on your insurance statement. If you have full coverage, there is no out-of-pocket cost. Our billing team is available to answer any questions about your individual coverage.
